<p><img src="http://assets.fiercemarkets.com/files/healthcare/fierceimages/drvillarin.jpg" alt="" hspace="5px" width="84" height="120" align="right" />With  the Centers for Medicare &amp; Medicaid Services (CMS) expanding its  non-reimbursement policy for hospital-acquired conditions (HACs) from  Medicare to Medicaid, the need for hospitals to minimize the incidence  of HACs has grown more urgent.</p>
<p>So-called nosocomial infections, which originate in a hospital, are  responsible for 1.7 million infections each year, resulting in 99,000  deaths and costing the U.S. health system $20 billion annually in extra  costs, according to the Centers for Disease Control and Prevention.  Although hand hygiene is the easiest and most effective measure to  prevent HACs and related deaths, hospitals struggle to attain even a 50  percent plus hand-washing compliance rate by healthcare professionals.</p>
<p>A major difference between RTLS and non-RTLS users is that the  former essentially trains a &#8220;mechanical eye&#8221; on people 24/7 rather than a  brief interval. With RTLS technology, every party knows whether a  clinician washed or failed to wash his hands <em>(Comment from PeterM. This is unequivocally false)</em>. The data are in black and  white and unbiased, leaving no doubt, which is not the case when human  monitors are involved. Unlike people, RTLS technology is immune to  distractions and can function without rest day after day.To be sure, a facility can&#8217;t expect a dramatic turnaround in  compliance simply by rolling out RTLS. A hospital&#8217;s ability to  successfully implement the technology, effect fundamental change and  influence people to alter behavior learned over the course of a long  career hinges on senior leaders delivering a loud and clear message to  the rank-and-file. Specifically, executive and physician leaders  including the CEO, president, department chairmen, and chief medical  officers must champion RTLS to clinicians and staff from the get-go.  They must explain that the technology is designed to help personnel  deliver safer care, follow best practices and act in patients&#8217; best  interest rather than get staff in trouble.</p>

<div>I wrote a short paper on how different technologies satisfy  hand-hygiene monitoring requirements (<a href="http://tinyurl.com/43akud9" target="_blank">http://tinyurl.com/43akud9</a>).</p>
<p>There are schools of thought that &#8220;any&#8221; technology that improves  hand-hygiene is a &#8220;win&#8221;, even increasing spend on secret-shoppers. At  the end of the day if you increase compliance, your net effect is a  lowering of HAI&#8217;S and bottom-line you save lives with a cost variable  based on how you go about it.</p>
<p>So, when it comes to hand-hygiene one might say that anything is better  than nothing. But commercial solution vendors should make sure that  customers and readers alike understand [unequivocally] that certain  technologies like RTLS cannot, will not and do not detect complaint or  defective hand-washing events. They are recording location and proximity  events, and sometimes correlating proximity with best guess timings,  and possibly manual breaking of a beam of light &#8211;  and all of these  complexities, taken either separate or in combination, have little to  nothing whatsoever to do with detecting if a health-care worker actually washed their hands.</p>
<p>If you believe, like the CDC, Joint Commissions and World Health  Organization does that &#8220;efficacy&#8221;; the capacity to produce an  effect, has anything to do with hand-washing, and/or detecting a minimum  parts per million of alcohol within a sanitizer and both in combination  have anything to do with detecting either a compliant or defective  hand-washing event, then you should also agree that RTLS systems do not  detect actual compliant or non-compliant hand-washing events.</p>
<p>While some organizations may be OK with using technologies that are &#8220;good enough&#8221; the reality is that without a high-degree of  certainty these same organizations cannot employ something simple such  as an employee incentive program for hand-hygiene compliance.</p>
<p>They really become hampered, by their own technology, in achieving the larger goal which is to change human behavior!</p>

<p>I&#8217;ve been selected for additional screening several times (both in Charlotte and in San Diego, two places which have made the news) and to be honest I just grin and bear it, as a seasoned business traveler I am trying to get someplace and the last thing I need or want to do is cause myself to be delayed, miss a flight and or a meeting.<span id="more-1177"></span></p>
<p>I did learn however why I was being selected for the additional screening so often, since the X-ray machine requires that you take off your belt (plus everything else in your pockets), something that I didn&#8217;t do going through the metal detectors, my pants were falling down, and the minute I put my hands in my pockets to keep my pants up, I triggered a TSA protocol for the additional screening. It didn&#8217;t make any sense to me, I just went through the X-Ray and you saw me butt naked, why do I now have to go through an invasive pat down? I could’ve understood the logic if I went through the metal detector. Then, one of the agents finally explained that the second I put my hands in my pocket, as I was stepping out of the X-Ray machine, it&#8217;s an automatic thing. It still doesn&#8217;t make any sense, but OK, at least my question was answered.</p>
<p>So don&#8217;t put your hands in your pocket or conversely put your hands in your pockets if you want to test the security protocol. Anyway, yes &#8211; the screenings were invasive and maybe it’s a male vs. female thing, but quite frankly I just didn’t care (as long as it’s just me). If I was traveling on leisure with my 10 year old daughter, I would not allow her to be “patted” down; my view point would be totally different, to the point where I would re-consider air travel all together.  But, for a lone road-warrior– it’s just one more thing – and I do keep in mind that the end goal is security, percentages of effectiveness aside, it is for security – so why would I be against that. Plus, we are at version 1.0, like software products version 2.0 will hopefully be more effective and more user friendly.</p>
<p>As a frequent business traveler I just want to get to my seat and get my roller board in the overhead, near my seat without having to swim upstream when the plane lands &#8211; so I don&#8217;t have time to get into debates with TSA agents when I travel – and actually I take a different approach, for the airports that I frequent often, you tend to see the same agents over and over, so I make sure I greet the TSA agents, say hello, get to know their names, and make a little small talk if at all possible, so they remember me the next time – the old adage you get more with honey than with vinegar still works. I certainly can&#8217;t afford to turn away and go back home when I’m traveling on business, so maybe I&#8217;m a bit taken hostage by the process and cavalier with the bigger issues, but I don’t think so.  The news talks about the beginnings of a police state, and such. Well, if you’ve ever lived in a shore community, where the night life gets a little wild with youth in the summer, then you have been overly subjected to the weekend DWI road blocks, quite the inconvenience if you are a local, but we put up with them and they are nothing more than legalized excuses to stop and scrutinize motorists for which there would otherwise be no reason to do so. And for how long have these been around, and don’t make a U-Turn to get off the line (or else).  Or, have you ever been stopped by a State Trooper, let’s be honest you know the protocol, “Yes Sir, No Sir, Yes Sir, Yes Sir, Thank you sir, Good day sir” I’ll take my ticket and shut up. So the TSA security pat down is tantamount to the beginnings of a Police State? HHmmmm.</p>
<p>All that said, I do disagree with many of the cable news pundits. I do believe the security screening is (capital letters IS) effective. To me it&#8217;s just common sense, if someone has bad intentions, and that bad intention is on their person, how could all of the hullabaloo taking place at the security entrance at the gate areas not make anyone think twice about going through? Is it a stop gap measure, just one plug in a large damm with many holes, OK, yeah I get it &#8211; put on your big boy pants stop the whining and just devise a better, smarter way to solve the problem and make a boat load of money in the process, isn&#8217;t that the American way.</p>
<p>Also, I&#8217;m not sure what the percentage of road warriors is to non-road warriors but I&#8217;ve heard that less than 4% of the traveling public are frequent fliers, which means that the lion share of the people chiming in on this debate are barely affected. Overall there seems to be two sides to the debate, the give an inch take a mile concern in terms of government intrusiveness vs. take all you want just keep me safe, perception vs. reality, whatever &#8211; safe is safe. Which side do us road warriors fall on this one?</p>

<p>I am extremely excited, humbled, and, candidly, torn by this  opportunity.</p>
<p>In early 2003, Andy Typaldos, Elias Typaldos and  Gerry Vendome the co-founders of AXS-One (formerly Computron Software)  and the then current CEO John Rade all took a chance on me. I was a  former IT executive (or Chief Bottle Washer) who left the corporate  world several years prior to join the dot-com ranks of start-ups and  entrepreneurs &#8211; after several ventures in management consulting,  e-commerce and storage management I was a storage and long-term archive or better said, a digital preservation, compliance and legal discovery evangelist and I spent most of my time advising companies, governments and  some academia types on how to keep large volumes of electronic data around and readable  for a very long time, typically the 100 year + category, along with all of the risks [or not] of doing so. I was also doing some work with  various start-up ventures helping them to differentiate their storage and data encryption security products  for the US storage market.</p>
<p>In 2003, when Andy (one of the original founders of the company) walked me into AXS-One the executive team gave me the opportunity to build a new commercial software product and business venture for enterprise scale archival,  compliance and legal discovery within a publicly traded 40Mil dollar a  year co. and steer it where it needed to go in order to grow a new world  wide software business. What they didn&#8217;t know, at the time, was that I had actually started my career working as a software developer for large law firms developing case management and discovery systems, something that would a few years later turn out to be a be a pretty good thing.<!--more--></p>
<p>My 5+ years at AXS-One were some of the best years of  my career, working with great people who have pushed me to work harder,  become smarter, and do better. I was fortunate to be in the middle  of a number of industry inflection points around storage, new cloud innovations, decreasing storage cost, huge data  volumes, major concerns around e-mail and electronic communications such as social media, new search technologies, increasing compliance mandates and a heated need for legal discovery &#8211; and we were able to  continually invent and build great software that “killed the  competition” and served our large enterprise customers needs, some before they or  even the industry knew they needed it. That’s real magic.</p>
<p>It’s  not every day that you get a chance to build new products in a newly  emerging software category and go head-to-head competitively with IBM,  EMC, Symantec, and a few other billion dollar entities, and beat them to  the punch regularly.  Our early success drove a <a title="Peter Mojica" href="http://www.csi1000.com/axs-one-2/" target="_blank">1200% increase in the  company’s stock value</a>, and drove new successive wins, quarter over  quarter, with Blue Chip accounts and quarter over quarter profitability.</p>
<p>And although I&#8217;ve been very fortunate to help the company to  grow, I also had a chance to experience first-hand its down-slide,  another type of learning experience that you don’t pick up in business  school.</p>
<p>A US economic down-turn that affected nearly every  business and that combined with some poorly executed management  decisions by a newly hired CEO eventually took a highly successful  nearly 300 person company with employees and customers in over 15  countries and a market cap that we grew from 12mil to nearly 100mil to  become an acquisition candidate in the fire-sale category &#8211; and while it  may have been nice to roll-up my sleeves and prove that it can be done  all over again, the cheese had moved and I realized it was time for  change.</p>
<p>I reached out to several friends and business contacts to  solicit their help in re-joining the likes of the bigger companies  (Microsoft, Cisco, a couple of big investment banks, EMC, EDS/HP,  Sun/Oracle etc.) and I was all but ready to pull the trigger on another  Chief Bottle Washer opportunity when I received a call from Awarepoint, a  company that I was already working with as an advisory board member,  and they were growing and looking to expand in several areas.</p>
<p>As a  member of their advisory board I was already impressed with the caliber  of their people and their tremendous value proposition, products and  overall market strategy.</p>
<p>Well, after meeting with them further I  felt right at home, these are bright, driven people and I was primed and  ready to join them in whatever capacity would work best for the  company, so here I am the newly hired VP of Product Management. It is an  opportunity to expand my experience and to work on some really hard and  varied problems; it&#8217;s also the opportunity to work again with some  fantastic people.</p>
<p>As the <a href="http://www.csi1000.com/wp-content/gallery/business_cards/awarepoint2.jpg">Vice President of Product Management</a>,  I  am tasked with aligning the company&#8217;s strategic goals with the delivery  of consistently great products, the optimization of day-to-day product  management, and extending Awarepoint&#8217;s market leadership position. Some  of these things are very familiar to me; others like being a new  vertical industry aren&#8217;t. That&#8217;s precisely the challenge I feel I need!</p>
<p>I  will remain living in Charlotte but will commute regularly to San  Diego, where most of the Awarepoint  executives are based.</p>
<p>A big  thanks, to all of my friends and business contacts who were willing ready and able to  help me re-join one of those <em>&#8220;big&#8221;</em> companies again and I won&#8217;t think any less  of you for staying holed up in your cushy corner offices, while I enjoy the never ending entrepreneur spirit to build the next billion dollar company!</p>
